1. Simply signed power of attorney (Power Form).

2. Description of the design in triplicate.

3. Three sets of the drawings using the metric system.

4. If convention priority is to be claimed, a copy of the home application must be submitted within three months of the filing date.

5. Details of the applicant.

1. Legalized power of attorney executed in the new name and/or address (Power Form).

2. Legalized certificate proving the change of name and/or address.

1. Power of attorney duly legalized as above (Power Form).

2. Merger document legalized up to the Kuwaiti Consulate
