Trademark/ Service Mark Requirements:

Simply signed power of attorney for each application (Power Form).
Fifteen printing block of the mark not exceeding 9 centimeters.
List of goods to be covered by the application.
No. and date of home registration, if any.
Certified copy of priority document must be submitted within a period of six months in case it is claimed.

Renewal of Trademark/ Service Mark Registrations:

Simply signed power of attorney for each application (Power Form).
Ten prints of the mark (even for word marks).
Late renewal is possible with 6 moths from expiration date with penalty.

Change of Name/ Address Requirements:

Simply signed power of attorney (Power Form).
Certificate of change of name notarized.
Number and date of the registered trademark.

Assignment Requirements:

Simply signed power of attorney (Power Form).
Deed of Assignment notarized. (Assignment Form)
Original Certificate of Registration for endorsement.
