  Trademark/ Service Mark Requirements:

  1. Simply signed power of attorney (Power From).

  2. Twenty prints of the mark (3 X 3 inches).

  3. Certified copy of the priority document in case priority is to be claimed.

  4. Details of the applicant.

  1. Power of attorney under new name (Power From).

  2. Official document attesting change of name.
